Request for Proposal CHAPTER 24 REGION-WIDE CRISIS ASSESSMENT SERVICES The Southeast lowa LINK (SEIL) region is requesting proposals for an entity to be the sole provider of Crisis Assessment for the Southeast lowa Link Region in accordance with lowa Code Chapter 24 as it pertains to the Accreditation of providers of services to persons with Mental Ilness, Intellectual Disabilities, or Developmental Disabilities. SEIL Region includes the counties of Des Moines, Henry, Jefferson, Keokuk, Lee, Louisa, Van Buren, and Washington. Crisis Assessment Services to begin on February 1, 2021. Calls for a Crisis Assessment can be made from Law Enforcement, Mobile Crisis, SEIL hospital emergency departments., SEIL Mental Health Centers, and SEIL management team. 1. Scope of Work: The scope of services that the provider must be prepared and qualified to provide is as follows: a. Identify which counties will be served. b. Develop a crisis assessment plan that will provide 24/7, 365 crisis assessment to the SEIL region by a licensed mental health professional. Assessment is defined per lowa Administrative Code 441.24 as: a face-to-face clinical interview to ascertain an individual's current and previous level of functioning, potential for dangerousness, physical health, and psychiatric and medical condition. The crisis assessment becomes part of the individual's action plan. c. Crisis Assessment staff will respond within 1 hour of contact. d. Crisis Assessment staff will provide either on-site or tele-connected assessments to determine the appropriate level of care. The assessment will be provided wherever the individual is located. e. Crisis Assessment staff will explain assessment results to the individual and family or guardian when appropriate. f. Crisis Assessment staff will complete an action plan that will include individual's strengths, preferences and needs. The family or guardian may receive a copy of the action plan with a signed release. g. Sufficient data shall be collected to determine the appropriate level of care for individuals served and provided to the SEIL region for analysis of utilization, effectiveness, and dispositional outcome, per the SEIL Management Plan. h. Crisis Assessment Provider will bill Medicaid within 60 days of service in accordance with Medicaid allowable reimbursement. II. Statement of Qualifications. Proposals to SEIL should include the minimum information: SEIL requires that Crisis Assessment provider of the intervention program to be Chapter 24 accredited. Verification of accreditation will be required with application or provided prior to July 1, 2021, if not currently accredited. Staff with licensed mental health professionals. Description of organizational capacity to complete necessary activities, including capacity to provide licensed mental health professionals on call 24 hours per day. 7 days a week, 365 days per year to provide program oversight and sufficient access to service. Names and professional experience of key personnel who will be assigned to this project. References from previous clients of related work within the past five years. · Ability to obtain agreements with law enforcement agencies in covered jurisdictions that the provider will be working with, indicating that agencies agree to participate in crisis assessment activities. Upon award of proposal, all requirements of the SEIL contract shall be met. SEIL contract template will be available upon request. II. Proposed Cost of Services - Proposals to SEIL should include the proposed cost to accomplish all scope of work for activities outlined above; including a budget for program implementation and annual program budget. IV. Evaluation Criteria - SEIL will evaluate and rank proposals received according to the following criteria: Experience with Crisis Assessment Previous Work Performance 25 Points 25 Points Capacity to complete Scope of Work 25 Points Proposed Cost 25 Points Total: 100 Points V.
